Default Vehicle System Malfunction

LONG POST BUT PLEASE PLEASE HELP!!!

I am officially at my wits end 😢 I bought this pre-owned 2015 Mazda CX-5 2.2 Akera from a dealership 1.5 years ago and love, love, LOVE her! Didn’t have a days problem until February this year when I sent her in for her 90 000kms service. At the service they performed all the standard : oil and filter changes etc... + did a software upgrade because of a recall released.

Coincidentally, a few days later, after receiving the vehicle back, it came up with this Vehicle System Malfunction fault. I returned the car to the dealer workshop where initially I was told the software upgrade could have glitched so it was reinstalled and the fault cleared. 2nd time same thing. 3rd time I was told to replace a Vacuum Sensor, so I did and fault cleared. A few days later more lights then appeared and disappeared and I was told to replace the exhaust gas pressure sensor, which I did (diagnostics were run everytime I took the car back)... eventually I was told the fuel injectors could be faulty, not working or have a carbon build up and quoted an obscene amount of money to strip the car to check. Ontop of the already huge amount of money I’ve fished out!

I then took my car to 2 fuel injector specialists who ran diagnostics and said there was nothing wrong with my injectors. The second specialists took my car to electronic specialists who ran diagnostics and said the fault was on the camera and ABS modules and because these faults don’t ever affect the performance of my car- they suspected this fault was coming up due to the software upgrade done at the service with Mazda. (More money spent btw)

Off to Mazda I went again and the software spec level from factory has been reloaded on on my vehicle so the car has returned to the same level as before they did the recall software update.... (vehicle set back to “as built factory settings”). I collected my car exactly a week ago and this morning the f$@^%*g fault is back!!!!

Someone has to be able to figure this out. I have copies of all the diagnostics run, an oil analysis (stating fuel dilution) and pictures of all the dash faults and dates they came up - if anyone needs any more info.

PLEASE, there has got to be some genius out there that can solve my problem before I go bankrupt or loose my mind.




Fault codes... each time a diagnostic is run a new / different code comes up.

Thumbs up Vehicle System Malfunction

Not sure if you’ve managed to solve your issue but I’ve recently had a similar issue on a CX30, I phoned the main dealer and was asked by the young lady on the other end “I’m not trying to be rude, but how dirty is your car?” 😳 she explained that it could be a problem with the sensors not being able to perform correctly if they were covered with dirt. I went to check and the car was in a fairly clean condition in my opinion, however the rear camera was a bit obscured so I cleaned it thoroughly, as a result the fault has disappeared 😀

That's funny right there!

I can't tell you for sure what the issue was/is with the OP's car (it's all about the diesel) but I can guarantee you that the issues are not caused by his/her car being too dirty.

Seems to be a fairly frequent occurrence on some forums. The OP has a problem and posts/asks for a solution. Help is offered and not even the courtesy of a response. Go figure.....

It’s very common on vehicle forums. The OP googles his/her problem, finds an appropriate vehicle forum, joins to ask his question, either solves his own problem elsewhere or forgets he posted - leaving us all hanging. Dredging up zombie threads like this one can be avoided if we simply check the date of the OP’s last post; if its been more than a month it’s probably best to avoid resurrecting the thread by responding.

The first response was filled with mistakes made by service people doing the work. This is not an opinion of mine let me be clear and can be read by the suppling documents in the thread.
The advise here is if anyone needs to have their ECU flashed to have whomever is doing the work to first create a back up file and save it! This way if you need to reinstall the ECU back to where it was *originally before the changes it can be easily done with a pass-through tool. I am NOT talking about flashing with a base line ECU program because the *original ECU may have had some differences in it that are not even in a factory ECU flash. In this case the OP's service people created a domino effect of what apparently began with bad ECU flash .

That supposes it can be saved; I can only speak for GM but it does not give me an option when reprogramming a module (with their software) to save the old file. Aftermarket I'm sure they can, but maybe not at the dealer. (Although jumping through hoops it might be possible to have GM "allow" me to use an older programming from them). Thankfully I only bricked one (new) module while I worked for them due to an internet glitch during programming.

You need to re-read my response.
Oh and yes aftermarket can as well independent service shops do memory and complete OS back ups. As I said re-read my response.
I actually traveled from California to Florida to attend and get certified as a EFI/ECU Performance Calibrator have had a business with it for about 15 years on 2 other platforms. But in the shop I perform ECU pass through when changing out a customers ECU and always make a back up file to hold for at least 12 months which is generally the extent of a rebuilt ECU warranty

Default OE programming Vs Aftermarket

The OP stated it was originally flashed at the dealer; I was pointing out that a dealer likely does not have a way to save the original programming. I'm quite aware of what can be done outside that environment and YES always backup before changing in that case especially if under warranty so it can be reverted before the dealer can peek at it. We had an ATS that went through a couple engines and some pistons under warranty and even saw a video of him at the dragstrip with it, but since it always came back in "stock" condition and programming it kept being warrantied. I fully support modifying the tune and I follow "FasterProms" (among others) on You Tube but it can void warranty if found out so be cautious. I can't say much since when my '84 Mazda GLC was finally out of warranty I went to free flow exhaust with no cat, and a pair of Weber 40 DCOE carbs with soft compound "Gatorback" tires... (I miss that car!)
