Why does my P0421 code come and go ?

Why does my P0421 code come and go ?
It seems every couple of months I get this code. I have a code reader and I just clear it.
The front cat and 02 sensor are fairly new so I wonder what it could be ?
Reading up on it, there are so many possibilities....
