If you do a search of this forum with A/C heater fan fan speed etc. you'll find stuff where owners ave had similar problems, with opinions about equally divided between the fan switch and the fan motor resistor bank being the culprit. Without stripping things down and doing the electrical measurements, you just can't make the call.
Since neither part is very expensive, both equally failure prone at your vintage, and both demand stripping out some trim in the same general area, in your situation I'd be tempted to replace both and rest content knowing that you will not have a recurrence of either a switch or a resistor bank problem for the likely life of the car.
