Mazda Radio code reset
I have found the solution:
switch your car in contact
push 1 by 1 the following key
6 - AM - AUTO-M - SEEK^
4 dashes appears
Enter code 4989 by using the select buttons 1-6
press and hold down the AUTO-M and 6 button until GOOD appears.
Now your carradio is from its anti-theft mode back to factory setting

Hi your post is really helpful, I have a Mazda Premacy 2005 and I only get four channels if I'm lucky, anyway I tried your Steps and stupid question lol but I'm oblivious to a lot of things??? Do I have the car on or off?? I know.. I done the steps and the radio just started scanning when i pressed seek up.. Sorry and thanks

My old CD / Radio died in my trusty TRIBUTE , but I managed to find a replacement at the wreckers for a fair price (but damn they're hard to remove !)
Then discovered when I hooked it up to my car that this unit had been programmed with a CODE - no chance of guessing it in 3 tries, and eventually got the ERR message
So your solution is priceless

Hi bought my mazda they changed the battery I tried to put cd in but won't open or work The radio works but need the cd player too I think it's in anti theft mode can anyone help.

Im going to hopefuly help alot of people out, but at the same time will upset Mazda dealers.I have the code to unlock Mazda radios the have 'ERR' due to 3 wrong codes. Do not listen to mazda book that states 3 wrong codes make CD playing Inop req replacement...
121 Metro DW10*2 > 203028, MX5 NB30P2, Milennia,323 BJ10*2, 626 GF10S2, Tribute. Step one:- press one at a time- preset 6, AM, Auto M, Seek ^ (up). Step Two:-Input this number, 4989. Step Three:- Press and hold Auto M and Preset 6 together. Step Four:- The Word 'GOOD' will appear on the screen indicated that the radio will work. Step Five:- Reprogram new Antitheft code into radio as per instrution manual.

My Mazda 5 2007 car radio was blocked ERR I tried everything with codes and pressing buttons but nothing worked. Until I held the round volume/on/off button pressed at the same time as the FM button. Hold for 10 seconds with the ignition key in the ACC position. After 10 seconds the blockage is lifted.
So my 2016 Mazda (with CD player) is exhibiting this issue - I just replaced the OEM battery, so I am assuming something needs to be flashed. 
I've read the entire thread, and most folks do not include their make, model, or year - so I haven't been able to pinpoint the process and codes for a newer CX-5.
Can anyone please summarize the process for a 2016 Mazda CX-5, sold in the US? THANK YOU!
