Welcome to the forum

This is sort of a difficult question to answer. Unfortunately, if the shop did not see or know why then all it would be to answer your questions is guessing all the suspected parts around the belt itself.
The first would be to post a detailed picture of your olde belt so we can see the damage wear and especially the inside ribbing.

*A general observance of belts I would have noted on any customers invoice have any under hood general inspection or service in our shop.

I had my serpentine belt replaced last month - with 46,000 miles on it. Normally, one might expect that to last at least 60k miles or more - but mine did have some wear on it.

They returned it to me and I keep it under the seat as a spare. The CX-30 also has a smaller belt for the A/C. The mechanic said mine still looked great and that it did not need replacing.

If you only have 14,500 miles on your car, that seems premature for a belt failure - but that was nice that it was covered under warranty.

P.S. Welcome to the forum!

I see you mis-read the first post as I did. Mine was in the miles (posted no comma so I read it as 145K miles ) yours was the belt was described as not failing but being "thrown" which suggest the tensioner or something was picked up in the pullies and allowed the belt to be "thrown" off the pulleys.

Good thought on getting a new one at your miles. And keeping the back up in the Mazda!

I had a belt thrown a few years ago by a small animal that crawled into the engine compartment and fell asleep. It happened upon start up. The garage said this was common, especially in the winter.

I did the same thing with the old serpentine belt on my 2004 F-150. It doesn't really take up any space and is "cheap insurance" in case you break down and the parts store is closed. Sure, I would still need a mechanic to install it - but at least it is with the vehicle.
