If this is a ticking noise, most likely the valves need to be adjusted. This is not uncommon if it has not been done in a while, and it will help it to run better.

if you've never adjusted valves b4 you need a 10mm wrench(dont hold me to the size tho) a flat blade screw driver and a feeler gauge. look in a chiltons or haynes manual for the correct clearance value and it should also tell you exactly how to do it

It's your lifters. They're gummed up. There two ways to go. You can change your oil to a heavier weight. (10w-40w) Which will increase your oil pressure. Or do a Marvel Mystery Oil flush. MMO flush is the long term cure, but more time consuming. MMO flush: Drain about a quart of oil off. Add a quart of MMO. Run your truck for about an hour (road and/or idle). Change your oil. It's going to come out like kerosene, super thin. Drive your truck for a couple of days with the new oil and then change it again. It takes at least TWO oil changes to get all of the MMO out of the system. PS, new filter for each change.

UPDATE; I just did a MMO oil flush on my 2600. I had a little valve slap at start up. Well my previous instructions didn't work as well on 161k mile B2600 as it did on my 218k mile SL Saturn. Here's the results: I dumped off about a quart oil. I added a quart of Marvel Mystery Oil. My lifters became quite immediately. I took it out for a drive around town for about 20 miles. When I got back to the house, I changed the oil and filter. After the change my lifters where ticking. It wasn't the hard start up slap, but still ticking. It was absolutely quite when I had the MMO in. So I dumped off a quart of the new oil and put back in another quart of MMO. It's back to quite and I going to leave the MMO in until my next 5k oil change. Do what works best for you as far as the oil change goes.
