Default 2014 Mazda 3 - Code P0171 - High LTFT

Hey everyone, I'm new to this car ownership thing and could really use some help. I recently became the owner of a 2014 Mazda Axela, which is basically the Japanese version of the Mazda 3. It has about 105,000 KM on it and I bought it used a little over a year ago.
So, a few months ago, I had a close call while driving home. I had to slam on the brakes so suddenly that my hazard light button got pushed in on its own (didn't even know that was possible!). When I got home, I noticed that my check engine light was on. I didn't think much of it at the time and since I didn't have an OBD scan tool, I just disconnected the car battery for a few minutes to try and reset everything.

A few days later, the light came back on. I went and got an OBDII scanner and it gave me the code P0171. I've been doing some research and trying to figure out the cause for the past three months.

Here's what I've done so far:
1. I did a smoke test on the engine to check for leaks, but found none.
2. I cleaned the MAF sensor and even had a mechanic swap in an old MAF sensor from another Mazda car.
3. I checked the Purge Valve for any vacuum leaks.
4. I also checked the PCV for any vacuum leaks.
5. As a test, I replaced the upstream O2 sensor with a cheap aftermarket one.
6. I even went as far as running three bottles of fuel cleaner through the engine over a two-month period to see if it would clear up any clogs.

Currently, the P0171 code comes and goes. It hasn't come back in about a month, but when I check the fuel trim using the OBDII scanner, my long term trims are stuck at +20, while my short term trim fluctuates between +5 at idle and around 0 at higher RPMs. I'm a bit worried that this might lead to more serious problems down the line.
Any advice or tips would be greatly appreciated!
