B2600 (2001) timing and stalling problems

I've just driven a 2001 B2600 (petrol) 4,000 km through hot outback Australia. I could feel the gearbox getting hot and the whole car felt overheated after a few days, but the radiator held up and the temp gauge sat solid as a rock at about 1/4 up from cold.
100 miles before I arrived, I noticed the car suddenly starting up with difficulty (had always started perfectly until then). When idling, the engine turned in surges or pulses of 1-2 seconds: higher revs, then lower. I managed to drive the rest of the way but it was skipping beats even at higher revs.
I've replaced the fuel filter and checked the spark plugs and leads but the problem has now got worse, not better (it's stalling more often now, after idling for just 30 secs or so).
Any ideas what could be causing this? Thanks in advance.
