weird in that it looked like the hole was a result a something rubbing against it, but nothing was close it. the hose also felt to be pretty thin too.
anyways, swapped it out & back in business!

That is amazing. I am having the exact same issue and the mechanics keep telling me they do not know what this is. I am not a mechanic but definitely handy. Is this a fix I should be able to do myself? I am assuming IM is Intake Manifold. As long as there is nothing major being removed I should be good. Thanks.

i haven't look at this post in awhile, but just now came across your note.
if you have the same problem that i had ( blow PCV hose ), this was a 1 minute fix.
the only other issue i came across since then was the sequential replacement ( over a +/- 6 month period ) of each coil pack( 6 total ). this threw a cel code ( i.e. P0301, P0303 ). depending on which one went out, determined how hard it was to fix. if it's any of the front located packs, then just removing the engine allowed access to the coil packs. if it's any of the back ones, then you need to remove the intake manifold too, along with the air intake hose, throttle body, egr valve.
not a hard job ( 30 - 40 min. ), but a pain in the axx.
lastly, if you replace any of the coil packs, i suggest going to any local auto parts house. they are have the price ( $65-$70 vs $140 @ mazda ).

in most cases is the air bypass valve
If you can't find it, go the dealer and look into the computer on the parts section
You can get also a free print out

I found a crack in the ribbed part of the pcv air intake hose (if you zoom in on the ribs you can see the crack). I did a temp fix with gorilla tape until my new one arrives in the mail. I cleared the 'running lean' codes and the check engine light has remained off.
