Sounds like head gasket problem, piston rings gone or engine management system failure. There are two ways this can fixed, either by removing the CAT and bashing all the crap out of it, or by replacing it. Removal and refitting of the CAT is relatively cheap, whereas replacing the CAT can be quite costly.
